BCBSIL Announces Legislative Proposal

Blue Cross and Blue Shield of Illinois (BCBSIL) today announced a new legislative proposal designed to help hundreds of thousands of the "working uninsured" in the state gain access to health care through affordable health insurance. The proposal, which requires all private health insurance carriers in the state to participate, would require qualified small businesses with between two and 50 employees - in addition to offering the currently mandated small group insurance coverage - to offer a basic health insurance option tailored to meet their needs.

"This proposal adopts an incremental approach that could have a major impact on a significant segment of our state's uninsured population, and at the same time, reduce uncompensated care," said Paul S. Boulis, BCBSIL president. "And it doesn't disrupt the existing health care system, which serves the 84 percent of Illinoisans who are insured." "One-size-fits-all proposals that totally overhaul the system have not proven to be cost effective," he added.

More than 1.7 million (16 percent) of Illinoisans don't have health insurance Some 16 percent, or more than 1.7 million Illinoisans do not have health insurance - approximately 1.1 million of them are either employed or are family members of the employed. A significant portion of the uninsured works for small businesses.

"Not having health insurance can be detrimental to people's personal and financial health and well-being," said Boulis. "Further, uncompensated care is a burden on individuals, employers, providers and others, because it increases health care costs overall. The proposed legislation would reduce uncompensated care by bringing new resources into the health care system."
Proposal for "working uninsured" in small businesses While the proposal would require small business owners to offer their employees basic health coverage, it would not require them to contribute to premiums. Those businesses that choose to contribute would be offered state tax credits. Means testing would ensure that people who need this benefit the most, the working uninsured, get it. Employees of businesses that qualify for the program would be guaranteed access to the new benefit option.

"We're offering a simple proposal that would help the state overall, individuals, small businesses and providers, in addition to the working uninsured," said Boulis. "We call upon members of the Illinois General Assembly to help make this proposal a reality in order to increase access to health care through affordable health coverage."

Mystery still exists in billing codes

House leaders on Wednesday removed a provision that would have required hospitals to make public some price information from a health care information technology bill (HR 4157), CQ Today reports (Crowley, CQ Today, 7/26). The legislation, sponsored by Reps. Nancy Johnson (R-Conn.) and Nathan Deal (R-Ga.), would codify the Office of the National Coordinator for Health Information Technology within HHS and would establish a committee to make recommendations on national standards for medical data storage and develop a permanent structure to govern national interoperability standards. The bill also would clarify that current medical privacy laws apply to data stored or transmitted electronically and would require the HHS secretary to recommend to Congress a privacy standard to reconcile differences in federal and state laws. Under the bill, the number of billing codes health care providers use to file insurance claims would increase from 24,000 to more than 200,000 by Oct. 1, 2009 (American Health Line, 6/8).

In addition, the legislation would provide grants of about $30 million for the implementation of electronic health record systems. According to CQ Today, the "price transparency" provision, supported by conservative lawmakers as part of an effort to "make the health care marketplace more amenable" to health savings accounts, "angered the hospital industry, which lobbied lawmakers to take it out of the bill." Some lawmakers also criticized the provision because the measure did not appear in versions of the bill passed in June by the House Ways and Means and Energy and Commerce committees. The removal of the provision "is likely to help smooth passage of the bill ... when it comes to the floor Thursday," CQ Today reports (CQ Today, 7/26). One House leader on Wednesday said that the legislation likely will pass (O'Connor et al., The Hill, 7/27).

To help control rising health care costs, Blue Cross and Blue Shield of Illinois (BCBSIL) has contracted with American Imaging Management (AIM) to implement a statewide utilization management and quality improvement program of high-tech outpatient diagnostic imaging services. Beginning April 3, a Radiology Quality Initiative (RQI) program will be implemented for elective, outpatient, high-tech imaging services - MRI, PET, CT, Magnetic Resonance Angiography (MRA) Scans and Nuclear Cardiology - for all members, except HMO, residing in Illinois and Lake County, Indiana.

The RQI program promotes appropriate ordering of outpatient diagnostic imaging services based upon the member's diagnosis, while minimizing unnecessary radiation exposure.
What does this mean?Effective for dates of service on or after April 15, 2006, all non-emergency imaging services listed above, when performed in a physician's office, outpatient department of a hospital or a freestanding imaging facility will require an RQI number.
How RQI WorksPhysicians ordering an imaging exam for the elective high-tech services must contact AIM prior to scheduling. The recommended service is evaluated to determine the appropriateness according to diagnosis. AIM will also direct the ordering physician to a contracting network provider as needed.

AIM will provide a consultative service to physicians, helping them select the most appropriate radiological service for their patients. Although physicians' orders will go through AIM's clinical review process, no denials will be given and no medical necessity decision will be rendered. The RQI process will be transparent to members.
